This form is used to report a loss or damage to University of Iowa owned property. Losses estimated to be greater than $5,000 must be reported to Risk Management no later than the next working day following the loss. Initiate emergency repairs to prevent further loss. Loss involving theft or other criminal activity should be reported immediately to Campus Safety (319)335-5022. A loss investigating theft of electronic devices also must be reported to it-security@uiowa.edu, (319) 335-6332.
