How to report incidents to Risk Management for losses on or off campus that cause damage or may result in a liability claim against the University of Iowa.
University of Iowa Property Damage and Losses
- Complete and submit a Property Loss Report to Risk Management to report incidents that damage University of Iowa property.
- If property damage is estimated to exceed $5,000, this form must be submitted to Risk Management within 24 hours of the loss.
University of Iowa Vehicle Accidents and Damages
- Complete an online Vehicle Accident Report via Employee Self Service or MyUI. Link to report accident is found under “Parking and Travel” tab.
- Report accidents with injuries immediately and all other accidents within 48 hours/2 working days.
Incidents on Campus that affect the Public, Students or Others
- Complete and submit an Incident Report Form to Risk Management for incidents that may result in a liability claim against the University.
Reporting Employee Injuries
- Complete and submit a First Report of Injury via Employee Self Service. Link to report injury is found under “General Systems and Tools” tab.
- Questions regarding Workers Compensation and employee injuries should be directed to University of Iowa Benefits. https://hr.uiowa.edu/support/workers-compensation
